Trying to run this in BFMulator 2.05 (latest) and even a mouse click on the slot doesn't clock up any credit...
OK, tried again just now and it seems to work - but new gripe:- can I change the S for Start to Space for Start?...

Tried unzipping it and leaving it zipped and even unzipping it into its own folder. It doesn't even appear when I scan on MFME version 2, 3 and 10.
Same happens with Cops and Robbers.
